Terms and Conditions
Confirmation of registration will be issued via email within 20 minutes of submitting this online form. Please contact forum@eventandconferenceco.com.au if you have not received a confirmation within this time frame. The registration form becomes a Tax Invoice once completed. Payment Full payment of registration fees must be received prior to the Forum. Unpaid delegates will be refused entry into the events, so please ensure that your fees are paid prior to your arrival at the Forum. Cancellations & Refunds All requests for registration refunds must be made in writing. For cancellations made prior to 12th July 2024, the registration fee minus a $150.00 per person processing fee will be refunded. Refunds will NOT be processed for cancellation after 12th July 2024, however substitute delegates are acceptable. No refunds available for the 2 person or 3 person packs. Registration Changes Any changes to your registration or optional bookings will not be verbally accepted. Please email your changes to forum@eventandconferenceco.com.au. Conditions Participation in The Forum and Trade Exhibition is at the sole discretion of the Pharmacy Guild of Australia WA Branch and is subject to conditions laid down by the Guild. The Guild reserves the right to refuse entry and this can occur without notice. Delegates intending to represent a company in any other manner than as a delegate to the Forum without approval from the organising committee will be refused entry. Condition of Entry It is a condition of entry to The Forum and Trade Exhibition that you may be photographed at the event, and that your image may be used on promotional material for future events. Disclaimer Participation is acknowledgement that no blame, liability or responsibility can be attributed to the Pharmacy Guild of Australia WA Branch, its staff or agents for any loss, damage or injury resulting from failure to carry out any undertakings or provision, or from substitution of any undertaking or provision. Transferability of Registration Should you be unable to attend any part of the Forum it is permissible to transfer your registration to another person. To do so, please visit the Forum registration desk or contact forum@eventandconferenceco.com.au. Please note that the above accommodation rates are a room only rate. Breakfast is available at an additional rate (from $27 pp depending on the hotel). Crown reserves the right to change the venue allocated for breakfast. Accommodation rooms may be allocated as a combination of king and twin rooms based on availability at the time of booking. If you require a particular room type please make a note in your booking. Rooms are allocated as Kings as standard. Check in is available from 15:00 and check out is at 11:00. If you would like to check in to your room prior to 15:00 Crown highly recommends that you book the night before for any early arrival requests to avoid disappointment. A full night’s accommodation charge will apply. Cancellation of Individual Bookings Should a decision be made to cancel individual bookings less than thirty (30) days prior to arrival or there are no shows, the below cancellation policy applies: Cancellation Policy- Less than 30 days including no shows Cancellation fee for the full length of stay cancelled will be charged to the applicable credit card.
